Informed consent/assumption of risk.
You have agreed to purchase a membership at GRAFT Strength that allows you access at any time. As such, you are aware that there will be NO SUPERVISION
OR ASSISTANCE DURING CERTAIN HOURS. You are also aware that if you are injured, become unconscious, suffer a stroke or heart attack, that there will likely be no one to respond to your emergency and this facility has no duty to provide aid to you.
You are agreeing to pay the monthly membership fee of £35 pcm via direct debit in return for access via a code lock door. Your membership number will be set as the code for the door and will work as long as the payments are maintained and you abide by the gym rules. Any additional services such as personal training must be booked through the staff and will come at an additional cost.
Even though this facility is equipped with surveillance cameras and panic phone, if it is likely that you should require immediate assistance, none will be provided. We HIGHLY recommend that you have a workout partner accompany you while at the club and keep your mobile phone handy at all times, but it is entirely up to you.
Because physical exercise can be strenuous and subject to risk of serious injury, the club urges you to obtain a physical examination from a doctor before using any exercise equipment or participate in any exercise activity. You (each member, guest, or participant) agree that if you engage in any physical exercise or activity, or use any club amenity on the premises or off premises including any sponsored club event, you do so ENTIRELY AT YOUR OWN RISK. You agree that you are voluntarily participating in the use of this facility AND ASSUME ALL RISKS of injury, illness, or death. We are also not responsible for loss of any of your personal property. Lockers have been provided for the safe stowage of personal effects.
This waiver and release of liability includes without limitation, all injuries which may occur, regardless of negligence, as a result of;(a) your uses of all amenities and equipment in the facility and your participation in any activity, class, program, personal training or instruction, (b) the sudden and unforeseen malfunctioning of any equipment and (c) your slipping and/or falling while in the club, or on the club premises, including adjacent parking areas and sidewalks
We may at times need to close the facility due to maintenance, staff training or work that needs carried out. We will email all customers to give notice of this but by accepting this you are agreeing that you understand that and accept and inconvenience it creates.
Sunbed use:
A suntan should be built gradually, particularly with sensitive skin. Never repeat UVA suntan treatment if any signs of redness remain from previous sunbathing. Wear goggles or approved protective eye wear, remove contact lenses, keep eyes closed and avoid looking directly at the light tubes. Do not tamper with or touch the light tubes whilst using equipment. Persons with sensitive skin or no existing suntan should restrict the first few sessions to a maximum of 6 minutes. It is also advised that body lotions, cosmetics, perfumes and deodorants should not be worn during treatment.
Although GRAFT Strength Offers one sunbed with the ability to have multiple sessions per week, health and safety guidelines state that no one person should have any more than 20 sessions per year and we strongly recommend that you follow these guides although you are not obliged to do so.
GRAFT Strength, their staff and agents assume no liability for persons who undertake sun tanning treatment, and if you are in any doubt after completing this form please consult your doctor.
Sunbed Indemnity:
Please fill out a paper questionnaire form from the reception desk in GRAFT CrossFit before purchasing any tokens. I confirm that to the best of my knowledge the answers to the questions above are correct. I have been advised that if I have answered YES to any questions in the first table I should consult my doctor before using sun-tanning equipment, if I answered yes to any questions in the second table that I should not use the sun tanning equipment at all. I have read and understood the statement above and I am totally satisfied that I am suitable for UVA sun tanning treatment.
